Using GIP

So, I've decided to start my own hospice blog to talk about the things I don't seem to be able to get out in my day job (or the hours I spend in the evening talking to my nurses!). I am a 27 year old social work associate in Washington State. I work part-time for our only local non-profit hospice in a semi-rural area. I currently oversee our nursing home program.

I'm currently having difficulty appropriately utilizing our inpatient facility for our nursing home residents at end of life. The new COPs make it necessary that we treat SNF residents equal to home hospice patients, but difficult for us to show that transfer is indicated at end of life from a skilled facility. The particular difficulty for us is that we have had a free standing hospice care center since the early 80s, so local residents are accustomed to the idea that patients will be "able" to transfer there at end of life.

Has anyone else come up against this? Have you prevailed in the face of the new COPs? Please post your help below
